We’re excited to begin preparing for the fall sports season! Athletic Clearance for Fall Sports will open on May 18, 2026. All student-athletes must be fully cleared prior to participating in any tryouts, practices, or competitions.

Fall Sports (Clearance Due: Friday, June 5, 2026)
Cross Country (Boys & Girls), Girls Tennis, Girls Flag Football, Boys Water Polo, Girls Field Hockey, Girls Volleyball, Girls Golf

How to Complete Athletic Clearance

• Visit www.athleticclearance.com
• Select California
• Create an account (parent/guardian email required) or log in
• Select “New Clearance” and choose Sage Creek High School for the 2026–27 school year
• Complete all required student information, medical history, and consent forms
• Athlete and parent/guardian must electronically sign all forms
• Upload the required documents:
  • CIF Physical Form (signed, stamped, and dated by MD/DO/PA/NP)
  • Proof of Medical Insurance
  • If a student uses any medication (prescription or over the counter) during the school day and/or while participating in sports, parent/student/prescribing health care provider must complete CUSD Medication and Procedures forms. These forms need to be completed annually per California State Ed Code.
• Once you receive an email confirmation stating your athlete has been CLEARED, the process is complete

• Walk-in only (no appointments).
• You may call the number on the flyer the day you plan to attend to be added to a waitlist (allows you to wait from home).
• Our dates are May 18th-May 29th from 11:00 AM-7:00 PM.

Please plan ahead to ensure your athlete is cleared by June 5th. Let us know if you have any questions!
